ON-SITE DAY PROGRAM SERVICES

Services empower individuals with skills and emotional support to remain in their community. A sense of personal worth and a sense of community are the "freedom landing" on which other skills are developed and enhanced. The ability to belong, make choices and decisions through problem-solving are the anticipated outcomes of group and unit participation. Individuals are involved in a variety of activities based on their needs and goals. Activities may include better management of personal hygiene, medical needs, financial planning, transportation, interpersonal skills, pre-vocational skills, and utilization of community services.

OFF-SITE COMMUNITY SUPPORT SERVICES

Based on need, services provide the individual with assistance in learning or re-learning daily living skills, which include: personal hygiene, cooking, housekeeping, bugeting/financial, and medication monitoring.

Eligibility

1.  18 years of age or older

2.  A psychiatric diagnosis that meets priority population within the Maryland Public Mental Health System (PMHS)

3.  Maryland Medicaid or proof of application

4.  Charles County resident; parts of Prince George's County i.e., Accokeek, Brandywine area up to and including Brandywine Road; parts of St. Mary's County i.e., Charolotte Hall, Golden Beach and Wicomico areas; or own transportation
