ON-SITE DAY PROGRAM SERVICES

Services empower individuals with skills and emotional support to remain in their community. A sense of personal worth and a sense of community are the "freedom landing" on which other skills are developed and enhanced. The ability to belong, make choices and decisions through problem-solving are the anticipated outcomes of group and unit participation. Individuals are involved in a variety of activities based on their needs and goals. Activities may include better management of personal hygiene, medical needs, financial planning, transportation, interpersonal skills, pre-vocational skills, and utilization of community services. On-site services are offered Monday, Tuesday, Thursday and Friday each week from 8:30 am to 1:00 pm.

OFF-SITE COMMUNITY SUPPORT SERVICES

Based on need, services provide the individual with knowledge based assistance in daily living skills, which include: management of personal hygiene, cooking, housekeeping, bugeting/financial, and medication monitoring.  Community support services are provided to individuals at hours determined by the individual and the facilitator of service.  Staff typically visit a minimum of once weekly, and depending on consumer need could visit more frequently.
